Type 2 Diabetes Group Services (Medicare)

We provide group services for clients with Type 2 Diabetes from our Caloundra clinic. Patients with Type 2 Diabetes may be eligible for a GP management plan through their doctor and receive a referral for Group Services for people with Type 2 Diabetes through Medicare. This referral entitles you to an initial assessment and 8 group services. These services are in addition to the 5 individual allied health services under a CDM plan. Following the initial assessment, you will be prescribed an individual program based on your current capacity and fitness level to be completed during the classes and education will be provided to all patients to help them better manage their condition.
